Comprehensive Independent Medical Opinions
Often Called a “Nexus Letter"

Psychological Independent Medical Examinations for VA Disability Claims

Performed by licensed psychologists to meet VA evidentiary standards, producing clear, clinically supported medical evidence for VA review. Each evaluation is tailored to the individual Veteran.

What a Nexus Opinion Is — And Why the Evaluation Behind It Matters

A nexus opinion is a medical conclusion connecting a condition to military service.

The evidentiary strength of that opinion depends on the quality of the clinical evaluation supporting it.

Our evaluations go beyond a basic nexus letter by including comprehensive clinical review, diagnostic assessment, and structured medical reasoning designed for VA review.

Every IME includes a nexus opinion.

Not every nexus letter reflects an IME-level clinical evaluation.

Each IME Includes:

A structured clinical process designed to produce clear, VA-compliant competent medical evidence for VA claims. Led by licensed PhD and PsyD psychologists employees of the practice, not contractors, ensuring consistent quality and a unified standard of care.

50–90 Minute Clinical Interview

Captures your full symptom picture, history, and lived experience for stronger evidence.

Review of all Relevant Records

Includes military service records and in-service / post-service medical treatment records to document critical detail.

DSM-5-TR Diagnostic Assessment

Provide the formal diagnosis VA adjudicators require (if clinically indicated).

Independent Medical Opinion

Provides a medical opinion supported by clear clinical rationale connecting your condition to military service or a service-connected condition when supported by the evaluation.

Feedback from a VA Disability Attorney

“As an attorney with 15+ years of experience in VA disability claims, I can say without hesitation that the team at Dr. Willoughby & Associates conducts thorough, effective, high quality independent medical exams relating to mental health conditions. Having reviewed numerous reports produced by them in connection with VA disability claims (including for primary service connection, secondary service connection, and increased ratings), it is clear they take the time to develop a thorough understanding and analysis of each client’s unique history and circumstances.

Dr. Willoughby & Associates further understands that what makes an effective exam report goes beyond facts and opinions – it also requires clear reasoning, a working knowledge of VA regulations and what the VA looks for from an evidentiary standpoint, and the ability to communicate medically-relevant facts, opinions, conclusions, and reasoning using language that bridges the gap between the worlds of mental health and the VA disability system. They understand what’s at stake for the Veterans with whom they work.

Although “attorney” and “integrity” are, these days, words one might not think go together, the reality is that anytime I refer someone to a medical provider for an independent medical exam, I am putting my own integrity and reputation on the line. I do not do that lightly, but Dr. Willoughby & Associates has earned my trust and respect through their competence, professionalism, and integrity.”

Comparing C&P Exam vs. Private IME Exam

C&P Exam

Clinician

⚠️ Often a VA‑contracted examiner (psychologists, psychiatrists, social workers, etc.) with variable veteran‑specific training.

Interview Length

⚠️ Often short, checklist‑driven, focused on functional criteria.

Report Depth

⚠️ Brief functional summary; not an IME – lacks detailed clinical interview; may not contain a DSM5TR diagnosis; or structured nexus opinion.

Turnaround

⚠️ Often several weeks to months before the report reaches the regional office.

Evidentiary weight

⚠️ May carry less weight if lacking diagnostic support or clear nexus rationale; additional evidence often requested resulting in
remand and extra delay.

Private IME

Clinician

✅ 50–90 minutes of in-depth clinical interview, allowing thorough discussion of your symptoms, service history, and how these experiences affect your daily life.

Report Depth

✅ Full narrative report with DSM‑5‑TR diagnostic assessment (if applicable), detailed clinical reasoning, and a nexus‑style opinion written to satisfy CFR regulations and standardsoptional DBQ available.

Turnaround

✅ Delivered within 7–10 business days of the completed interview.

Evidentiary weight

✅ Structured to meet 38 CFR Parts 3 and 4 evidentiary standards and typically treated as independent, competent medical evidence by VA adjudicators.

Frequently Asked Questions

We’re here to make your evaluation process simple and transparent from the start. Call us, at (443) 379-0447 and we'll answer any questions you may have.

What veterans often call a “nexus letter” is already included in every Independent Medical Examination (IME) we provide. Each IME contains a full nexus-style opinion section — the VA-accepted equivalent of a nexus letter — as part of a comprehensive, VA-compliant report.

This opinion is based on a 50–90-minute clinical interview, a DSM-5-TR diagnostic assessment (if clinically indicated), and a detailed medical rationale that directly connects your condition to your service. That’s exactly the type of competent medical evidence VA adjudicators look for when deciding disability claims and appeals.

We believe veterans deserve clarity from the start. Before you commit, you’ll receive a transparent breakdown of costs, exactly what’s included, and the scope of your examination. Every IME includes a full nexus-style opinion structured to meet CFR §3.159 evidentiary standards, with no hidden fees. A DBQ can be added as an optional extra if you choose. Please note that services are private-pay and not covered by insurance.

Yes. A private IME by a licensed psychologist can be submitted as evidence to the VA and often provides much stronger support than a C&P exam.

Most reports are completed within 7‑10 business days, depending on case complexity and documentation.
